Nurse Assessment
A registered nurse offers a complimentary in-home client evaluation and home safety check.
- A personalized care plan is catered to the unique requirements of each client.
- Suggestions are made for home safety enhancements.
- The client's risk of falls and hospital admissions are evaluated with recommended preventive actions.
Assigned Care Specialist
You will have a care specialist overseeing your care. They will familiarize themselves with your care requirements, ensuring we consistently meet or surpass your expectations.
Matching
Using our exclusive matching system, Philia's Care Coordinator handpicks the Care Team based on capabilities, character, and other prerequisites.
Periodic Check-ins
Scheduled follow-ups are performed starting the day after care initiation and no less than every 30 days to oversee our quality of care.
Care Quality Optimization
The Care Coordinator may identify training needs or swap a Care Team Member as required to maximize care quality.
Care Team Development Program
Our Care Team Members have growth plans that highlight their desired career progression and areas of enhancement. Our training program aids their development with specialized training, provided to equip them with skills tailored to their clients’ specific needs.
