How Philia helps to prevent falls:
- Support ambulation and/ or transferring to/ from a wheelchair
- Encourage and assist with strength and balance exercises
- Provide personal care such as bathing and dressing
- Perform household tasks such as laundry or other housekeeping.
- Identify and eliminate fall hazards in the home
Philia caregivers are provided specialized training on how to care for fall-risk clients.
Philia's Care Process
Nurse Assessment
A Registered Nurse provides a complimentary in-home client assessment and home safety inspection
- A care plan is created and tailored to the unique needs of each client.
- Home safety improvements are recommended.
- The client's fall and hospital admission risks are assessed. Mitigation actions are recommended.
Assigned Care Specialist
You will be assigned a Care Specialist to coordinate care. This person will get to know you and your care needs and will ensure we are meeting or exceeding your expectations.
Matching
Using our proprietary matching system, Philia's Care Specialist selects the Care Team based on skills, personality and other requirements.Complimentary In-Home Registered Nurse Assessments
Periodic Check-ins
Periodic Check-ins are done starting the first day after care begins and no less than every 30 days to monitor our care quality.
Care Quality Optimization
The Care Specialist may identify training needs or switch out the Care Team Member as needed to optimize care quality.
Care Team Development Program
Our Care Team Members have professional development plans that identify their career growth aspirations and areas that need improvement. Our training program facilitates their development and speciality training is provided to help them with the specific needs of their clients.
