Philia's Care Approach

Nurse Assessment

A licensed nurse conducts a complimentary in-home client evaluation and home safety check:

  • A customized care plan is formulated according to each client's distinctive requirements.
  • Recommendations are made for enhancements to home safety.
  • The client's susceptibility to falls and hospitalizations is evaluated. Appropriate preventative measures are suggested.

Assigned Care Specialist 

A dedicated Care Coordinator is appointed to supervise the care process. This individual will familiarize themselves with you and your care requirements, ensuring that we are fulfilling or surpassing your expectations.

Matching

With the aid of our exclusive matching algorithm, Philia's Care Coordinator selects the Care Team based on abilities, personality traits, and additional prerequisites. Complimentary in-home assessments by registered nurses are part of this process.

Periodic Check-ins

Routine follow-ups begin the day after the commencement of our Parole team’s cancer care program and occur at a minimum every 30 days to oversee the quality of care.

Care Quality Optimization

The Care Coordinator has the authority to identify areas for additional training or rotate Care Team Members as necessary to enhance the quality of care.

Care Team Development Program

Our Care Team Members are privy to professional growth plans that pinpoint their career aspirations and areas requiring enhancement. Our training program enables their progression and specialized training is given to equip them with the skills needed for their clients' specific needs.
